    Hantavirus Expansion Trends in Natural Host Populations in Brazil.

    Hantaviruses are zoonotic agents responsible for causing Hantavirus Cardiopulmonary Syndrome (HCPS) in the Americas, with Brazil ranking first in number of confirmed HCPS cases in South America. In this study, we simulate the monthly spread of highly lethal hantavirus in natural hosts by conjugating a Kermack-McCormick SIR model with a cellular automata model (CA), therefore simultaneously evaluating both in-cell and between-cell infection dynamics in host populations, using recently compiled data on main host species abundances and confirmed deaths by hantavirus infection. For both host species, our models predict an increase in the area of infection, with 22 municipalities where no cases have been confirmed to date expected to have at least one case in the next decade, and a reduction in infection in 11 municipalities. Our findings support existing research and reveal new areas where hantavirus is likely to spread within recognized epicenters. Highlighting spatial-temporal trends and potential expansion, we emphasize the increased risk due to pervasive habitat fragmentation and agricultural expansion. Consistent prevention efforts and One Health actions are crucial, especially in newly identified high-risk municipalities.

    Malaria Risk Drivers in the Brazilian Amazon: Land Use-Land Cover Interactions and Biological Diversity.

    Malaria is a prevalent disease in several tropical and subtropical regions, including Brazil, where it remains a significant public health concern. Even though there have been substantial efforts to decrease the number of cases, the reoccurrence of epidemics in regions that have been free of cases for many years presents a significant challenge. Due to the multifaceted factors that influence the spread of malaria, influencing malaria risk factors were analyzed through regional outbreak cluster analysis and spatio-temporal models in the Brazilian Amazon, incorporating climate, land use/cover interactions, species richness, and number of endemic birds and amphibians. Results showed that high amphibian and bird richness and endemism correlated with a reduction in malaria risk. The presence of forest had a risk-increasing effect, but it depended on its juxtaposition with anthropic land uses. Biodiversity and landscape composition, rather than forest formation presence alone, modulated malaria risk in the period. Areas with low endemic species diversity and high human activity, predominantly anthropogenic landscapes, posed high malaria risk. This study underscores the importance of considering the broader ecological context in malaria control efforts.

    The risk of vector transmission of Trypanosoma cruzi remains high in the State of Paraná.

    BACKGROUND: Monitoring and analysing the infection rates of the vector of Trypanosoma cruzi, that causes Chagas disease, helps assess the risk of transmission. OBJECTIVES: A study was carried out on triatomine in the State of Paraná, Brazil, between 2012 and 2021 and a comparison was made with a previous study. This was done to assess the risk of disease transmission. METHODS: Ecological niche models based on climate and landscape variables were developed to predict habitat suitability for the vectors as a proxy for risk of occurrence. FINDINGS: A total of 1,750 specimens of triatomines were recorded, of which six species were identified. The overall infection rate was 22.7%. The areas with the highest risk transmission of T. cruzi are consistent with previous predictions in municipalities. New data shows that climate models are more accurate than landscape models. This is likely because climate suitability was higher in the previous period. MAIN CONCLUSION: Regardless of uneven sampling and potential biases, risk remains high due to the wide presence of infected vectors and high environmental suitability for vector species throughout the state and, therefore, improvements in public policies aimed at wide dissemination of knowledge about the disease are recommended to ensure the State remains free of Chagas disease.

    Planning for an offshore oiled wildlife response: case studies from New Zealand and Brazil.

    When an offshore oil spill occurs, it is often assumed that there will be no wildlife impacted or that an oiled wildlife response could not be undertaken. In most cases, one or both assumptions are wrong. With increasing offshore fishing, petroleum exploration, and shipping routes, the risk of accidents and spills offshore has increased. This review outlines the important considerations for offshore oiled wildlife response and explores two case studies on offshore oiled wildlife response planning based on offshore drilling or active platforms in New Zealand and Brazil. There are significant challenges for running a response in offshore environments; however, with planning, including preparation of specialized response plans, equipment, and readiness of skilled personnel, an offshore oiled wildlife response can lead to greater survival and protection for wildlife and the environment.

    Translation, adaptation, and cross-cultural validation into Brazilian portuguese of the hearing protection assessment questionnaire (HPA)

    PURPOSE: The aim of the present study is to translate, adapt, and cross-culturally validate the Brazilian Portuguese version of the questionnaire Hearing Protection Assessment Questionnaire (HPA). METHODS: The original instrument, developed in English, seeks to assess barriers and supports related to the use of hearing protection devices (HPD), as well as workers' knowledge, habits and attitudes towards occupational noise. The translation, adaptation, and cross-cultural validation of the questionnaire consisted of five steps: Translation of the questionnaire from English to Portuguese; 2) Reverse translation from Portuguese to English; 3) Analysis of the instrument by three experts in the field; 4) Pre-test of the questionnaire with ten workers; 5) Application of the instrument to 509 workers in a meatpacking industry after the pre-employment medical exam. RESULTS: The results indicate the construction and content validity of the Brazilian Portuguese version for use with a working population and its internal consistency. CONCLUSION: This study resulted in the translation, cultural adaptation, and validation of the Hearing Protection Assessment Questionnaire (HPA), in order to be used to assess the use of individual hearing protection in the occupational field, called Hearing Protection Assessment Questionnaire (HPA).

    Apparent prevalence and risk factors for bovine tuberculosis in the state of Paraná, Brazil: an assessment after 18 years since the beginning of the Brazilian program

    Bovine tuberculosis (bTB) impacts considerably animal production and one health worldwide. To describe the prevalence, risk factors, and spatial pattern of the disease in the state of Paraná, Brazil, a cross-sectional study was conducted from September 2018 to February 2019. The area was divided into seven regions. Within each region, farms were randomly selected, and a predetermined number of cows was selected and tested by a comparative cervical tuberculin test. 17,210 animals were tested across 1757 farms. Herd prevalence of bTB-infected herds in Paraná was 2.5% [1.87-3.00%]. It has varied from 0.8 to 3.98% among seven regions, with clustering being detected in the west, central, and northeast areas. Animal prevalence was 0.35% [0.21-0.59%] and has varied from 0.08 to 0.6% among the pre-set regions. No major shifts in the prevalence of bTB were detected since 2007. Large-sized herds, dairy production, and feeding with whey were detected to be correlated with the presence of bTB. Exclusively among dairy herds, veterinary assistance from cooperatives, possession of self-owned equipment to cool milk, and feeding with whey were correlated with the disease. Considering these results, it is recommended that the state of Paraná seek to implement a surveillance system for the detection of bTB-infected herds transforming them into free ones, if possible, incorporating elements of risk-based surveillance. Health education is also recommended to inform farmers about the risks of introducing animals without testing and of feeding raw whey to calves.

    Children with HIV: A scoping review of auditory processing skills

    IIntroduction Auditory processing disorders can negatively affect academic performance in children. They can result from a number of aetiologies, including the human immunodeficiency virus (HIV). Although studies in paediatrics are limited, research suggests that HIV-infected children display poorer auditory processing skills than uninfected children. Methods The aims of this study were to scan the peer-reviewed literature on auditory processing skills in HIV-infected children, to describe how auditory processing was tested, how auditory processing skills were reported, and to identify gaps in current evidence. This systematic scoping review was conducted using a modified version of Arksey and O’Malley’s framework. Key words comprised ‘HIV’, ‘auditory processing’, ‘hearing’ and ‘child’. Electronic databases were searched for relevant articles published from 1 January 2000 to 30 April 2018, and reference lists of included studies were pearled. Two researchers reviewed the articles and extracted data on sample descriptors, auditory processing testing procedures, and auditory processing skills. A third author collated the results and resolved discrepancies. The American Speech-Language-Hearing Association description of auditory processing skills framed the analysis. Results Five articles were included in this review (three from Brazil, one each from Mexico and Tanzania). Samples, and methods of testing were heterogeneous. Three studies reported on localization abilities, while gap detection thresholds, performance on dichotic tasks and speech discrimination scores were reported in one article each. No one study tested all areas of auditory processing skills and there was limited information about the auditory processing skills required for learning. Conclusion This review highlighted the current sparse evidence-base for auditory processing in HIV-infected children. It identified the need to standardise testing procedures, measures of auditory processing skills, and sample selection.

    Asthma inflammatory phenotypes on four continents: most asthma is non-eosinophilic

    BACKGROUND: Most studies assessing pathophysiological heterogeneity in asthma have been conducted in high-income countries (HICs), with little known about the prevalence and characteristics of different asthma inflammatory phenotypes in low-and middle-income countries (LMICs). This study assessed sputum inflammatory phenotypes in five centres, in Brazil, Ecuador, Uganda, New Zealand (NZ) and the United Kingdom (UK). METHODS: We conducted a cross-sectional study of 998 asthmatics and 356 non-asthmatics in 2016-20. All centres studied children and adolescents (age range 8-20 years), except the UK centre which involved 26-27 year-olds. Information was collected using questionnaires, clinical characterization, blood and induced sputum. RESULTS: Of 623 asthmatics with sputum results, 39% (243) were classified as eosinophilic or mixed granulocytic, i.e. eosinophilic asthma (EA). Adjusted for age and sex, with NZ as baseline, the UK showed similar odds of EA (odds ratio 1.04, 95% confidence interval 0.37-2.94) with lower odds in the LMICs: Brazil (0.73, 0.42-1.27), Ecuador (0.40, 0.24-0.66) and Uganda (0.62, 0.37-1.04). Despite the low prevalence of neutrophilic asthma in most centres, sputum neutrophilia was increased in asthmatics and non-asthmatics in Uganda. CONCLUSIONS: This is the first time that sputum induction has been used to compare asthma inflammatory phenotypes in HICs and LMICs. Most cases were non-eosinophilic, including in settings where corticosteroid use was low. A lower prevalence of EA was observed in the LMICs than in the HICs. This has major implications for asthma prevention and management, and suggests that novel prevention strategies and therapies specifically targeting non-eosinophilic asthma are required globally.

    Utility of mosquito surveillance data for spatial prioritization of vector control against dengue viruses in three Brazilian cities

    BACKGROUND: Vector control remains the primary defense against dengue fever. Its success relies on the assumption that vector density is related to disease transmission. Two operational issues include the amount by which mosquito density should be reduced to minimize transmission and the spatio-temporal allotment of resources needed to reduce mosquito density in a cost-effective manner. Recently, a novel technology, MI-Dengue, was implemented city-wide in several Brazilian cities to provide real-time mosquito surveillance data for spatial prioritization of vector control resources. We sought to understand the role of city-wide mosquito density data in predicting disease incidence in order to provide guidance for prioritization of vector control work. METHODS: We used hierarchical Bayesian regression modeling to examine the role of city-wide vector surveillance data in predicting human cases of dengue fever in space and time. We used four years of weekly surveillance data from Vitoria city, Brazil, to identify the best model structure. We tested effects of vector density, lagged case data and spatial connectivity. We investigated the generality of the best model using an additional year of data from Vitoria and two years of data from other Brazilian cities: Governador Valadares and Sete Lagoas. RESULTS: We found that city-wide, neighborhood-level averages of household vector density were a poor predictor of dengue-fever cases in the absence of accounting for interactions with human cases. Effects of city-wide spatial patterns were stronger than within-neighborhood or nearest-neighborhood effects. Readily available proxies of spatial relationships between human cases, such as economic status, population density or between-neighborhood roadway distance, did not explain spatial patterns in cases better than unweighted global effects. CONCLUSIONS: For spatial prioritization of vector controls, city-wide spatial effects should be given more weight than within-neighborhood or nearest-neighborhood connections, in order to minimize city-wide cases of dengue fever. More research is needed to determine which data could best inform city-wide connectivity. Once these data become available, MI-dengue may be even more effective if vector control is spatially prioritized by considering city-wide connectivity between cases together with information on the location of mosquito density and infected mosquitos.
